What Went Wrong? Case Histories of Process Plant Disasters and How They Could Have Been Avoided (E-Book)
In addition to updating with incident descriptions and references from mostly the last 10 years, this version is different in two major ways. First, I have reorganized the writing into seven main sections (including a definitive introduction and conclusion containing new material) rather than two parts (A and B). While reading the fifth edition in 2017— when I began the revision process in earnest— I had the sense that our collective understanding of process incident causation was now clearly focused on management system improvements needed to address immediate technical causes. An integrated approach to consideration of causal factors therefore seemed appropriate. Several colleagues and the anonymous reviewers of my book proposal agreed that reorganization after five previous editions of growth would be valuable.
